:: How to Submit TEAM BASKETS ::

Once you have selected your team basket theme on the Sign-Up Genius, collected items and created your basket, submit all the details below! Please remember to include the name of your basket, list of included items and total value. All baskets and donations should be labeled with your team name and delivered to Elevate NW Woodinville between October 20th - 31st*.
